Transplant Unit

Remember to take the following items home with you after your renal transplant

• A week's supply of drugs.
• Your list of the drugs including the drug names, doses and how often they should be taken.
• Your copy of the discharge letter sent to your GP. You will need this if you see your GP or district nurse before they receive the same letter by post.
• Your discharge information booklet.
• Clinic appointment card with next appointment date and time completed.
• Two 24 hour urine collection containers, and one white topped urine specimen bottle, all labelled with your name and hospital number.
• Wound dressings for the first dressing change by the district nurse or for you to change yourself if you can do this easily.
